ABSTRACT

Abstract Objective This study analyzes the role of clinical simulation in internal medical residency programs (IMRP) in Obstetrics and Gynecology (OB/GYN), attributed by the supervisors, in the training of residents in the city of São Paulo (SP). Methods Cross-sectional descriptive, qualitative, and exploratory approach. Semi-structured interviews were performed with ten supervisors of Medical Residency programs in Obstetrics and Gynecology. Interviews were analyzed by means of content analysis under the thematic modality, starting with the core the role of clinical simulation in Obstetrics and Gynecology Medical Residency Programs. Results Supervisors view Clinical simulation as: a complementary tool for the teaching and learning process, a possibility of a safe teaching and learning environment, an opportunity to learn from mistakes, a support for professional practice committed to patient safety, a learning scenario for teamwork, a scenario for reflection on the work process in Obstetrics and Gynecology, a scenario for evaluative processes in the medical residency. Still according to supervisors, Clinical Simulation favors decision-making and encourages the resident participation in activities. Conclusion Supervisors recognize Clinical Simulation as a powerful pedagogical tool in the learning process of resident doctors in Obstetrics and Gynecology Residency Programs.


Resumo Objetivo O presente estudo analisa o papel da Simulação Clínica em programas de Residência Médica (PRM) de Obstetrícia e Ginecologia, atribuído pelos supervisores, na formação do residente no município de São Paulo (SP). Métodos Abordagem qualitativa, transversal, de natureza exploratória e descritiva. Foram realizadas entrevistas semiestruturadas com dez supervisores de programas de Residência Médica de Obstetrícia e Ginecologia. Para análise das entrevistas foi realizada análise de conteúdo na modalidade temática partindo do seguinte núcleo: o papel da simulação clínica nos Programas de Residência Médica de Obstetrícia e Ginecologia. Resultados A Simulação Clínica, na visão dos supervisores, emerge como: ferramenta complementar para o processo de ensino e aprendizagem; possibilidade de um ambiente de ensino e aprendizagem seguro; possibilidade de aprendizagem a partir do erro; suporte para prática profissional comprometida com a segurança do paciente; cenário de aprendizagem para o trabalho de equipe; cenário de reflexão sobre o processo de trabalho em Obstetrícia e Ginecologia; favorecimento na tomada de decisão; cenários de processos avaliativos na residência; e, por fim, estímulo à participação dos residentes nas atividades. Conclusão Os supervisores reconhecem a Simulação Clínica como uma ferramenta pedagógica potente no aprendizado dos médicos residentes em Obstetrícia e Ginecologia.

ABSTRACT

A pandemia de covid-19 provocou intensas mudanças no contexto do cuidado neonatal, exigindo dos profissionais de saúde a reformulação de práticas e o desenvolvimento de novas estratégias para a manutenção da atenção integral e humanizada ao recém-nascido. O objetivo deste artigo é relatar a atuação da Psicologia nas Unidades Neonatais de um hospital público de Fortaleza (CE), Brasil, durante o período de distanciamento físico da pandemia de covid-19. Trata-se de estudo descritivo, do tipo relato de experiência, que ocorreu no período de março a agosto de 2020. No contexto pandêmico, o serviço de Psicologia desenvolveu novas condutas assistenciais para atender às demandas emergentes do momento, como: atendimento remoto; registro e envio on-line de imagens do recém-nascido a seus familiares; visitas virtuais; e reprodução de mensagens de áudio da família para o neonato. Apesar dos desafios encontrados, as ações contribuíram para a manutenção do cuidado centrado no recém-nascido e sua família, o que demonstra a potencialidade do fazer psicológico.(AU)


The COVID-19 pandemic brought intense changes to neonatal care and required health professionals to reformulate practices and develop new strategies to ensure comprehensive and humanized care for newborn. This study aims to report the experience of the Psychology Service in the Neonatal Units of a public hospital in Fortaleza, in the state of Ceará, Brazil, during the social distancing period of the COVID-19 pandemic. This descriptive experience report study was conducted from March to August 2020. During the pandemic, the Psychology Service developed new care practices to meet the emerging demands of that moment, such as remote care, recordings and online submission of newborns' pictures and video images for their family, virtual tours, and reproduction of family audio messages for the newborns. Despite the challenges, the actions contributed to the maintenance of a care that is centered on the newborns and their families, which shows the potential of psychological practices.(AU)


La pandemia de la COVID-19 ha traído cambios intensos en el contexto de la atención neonatal, que requieren de los profesionales de la salud una reformulación de sus prácticas y el desarrollo de nuevas estrategias para asegurar una atención integral y humanizada al recién nacido. El objetivo de este artículo es reportar la experiencia del Servicio de Psicología en las Unidades Neonatales de un hospital público de Fortaleza, en Ceará, Brasil, durante el periodo de distanciamiento físico en la pandemia de la COVID-19. Se trata de un estudio descriptivo, un reporte de experiencia, que se llevó a cabo de marzo a agosto de 2020. En el contexto pandémico, el servicio de Psicología desarrolló nuevas conductas asistenciales para atender a las demandas emergentes del momento, tales como: atención remota; grabación y envío em línea de imágenes del recién nacido; visitas virtuales; y reproducción de mensajes de audio de la familia para el recién nacido. A pesar de los desafíos encontrados, las acciones contribuyeron al mantenimiento de la atención centrada en el recién nacido y su familia, lo que demuestra el potencial de la práctica psicológica.(AU)

ABSTRACT

L'évaluation de la satisfaction des patientes est une composante essentielle de l'amélioration de la qualité des services en anesthésie. Notre objectif était de déterminer les bénéfices/risques de la Lidocaïne par rapport à la gestion de la douleur post-césarienne et son coût dans le but d'améliorer la prise en charge des patientes douloureuses. Méthodes : Nous avons procédé à une d'une étude prospective comparative randomisée sur une période de dix mois au sein du Centre Hospitalier Universitaire Gynécologie Obstétrique de Befelatanana. La population d'étude a concerné les femmes opérées pour césarienne sous rachianesthésie. Nous avons évalué la douleur avec la règle EVA (0 à 100 mm) à H1, H2, H3, H4, H8, H12 postopératoire. Résultats : Nous avons inclus 40 patientes pour le groupe A avec Lidocaïne, et 40 patientes pour le groupe B sans Lidocaïne. L'intensité moyenne de la douleur à H12 postopératoire, est de 17,6 ± 11,5 mm pour le groupe A et 29,1 ± 7,4 mm pour le groupe B, avec une différence statistique significative entre les deux groupes (p = 0,008). Conclusion : L'infiltration pariétale de Lidocaïne en peropératoire, pour la prise en charge de la douleur post-césarienne, est bénéfique, comparée au traitement standard seul

ABSTRACT

Objetivo: Evaluar la percepción de las madres que tuvieron su parto durante la pandemia COVID en relación a la obligación de no recibir visitas en su puerperio, y cuantificar la frecuencia de prematuridad que otros centros del mundo mostraron que se redujo durante la pandemia. Método: Estudio observacional retrospectivo con encuestas realizadas entre el 1 de septiembre y el 31 diciembre 2020, y análisis de estadísticas locales de la Unidad de Maternidad y Neonatología. Resultados: Sobre el 90% de las madres que contestaron la encuesta afirmó haber descansado mejor y preferir un retorno a las visitas con horarios reducidos. La frecuencia de prematuridad se redujo significativamente en nuestra institución (8,08% entre 2014 y 2019 vs. 1,6% en 2020). Conclusiones: La mayoría de las puérperas prefiere un horario reducido para visitas en el posparto. Este hallazgo y la caída en la frecuencia de prematuridad obligan a reflexionar sobre nuestro cuidado prenatal actual.


Objective: To evaluate the perception of mothers who gave birth during the COVID pandemic in relation to the obligation not to receive visits during the puerperium, and to quantify the frequency of prematurity that other centers in the world showed decreased during the pandemic. Method: Retrospective observational study with surveys conducted between September 1st and December 31, 2020, and analysis of local statistics from the Maternity and Neonatal Unit. Results: Over 90% of the mothers who answered the survey stated that they had rested better and preferred a return to visits with reduced hours. The frequency of prematurity was signficantly reduced in our institution ((8.08% between 2014 and 2019 vs 1.6% in 2020). Conclusions: Most postpartum women prefer a reduced schedule for pospartum visits. This finding and the drop in the frequency of prematurity force us to reflect on our current prenatal care.

Resumen Objetivo: La pandemia de SARS-CoV-2 ha obligado a una reorganización de las visitas presenciales, y por ese motivo se han minimizado hasta el punto de reconsiderar la realización de la visita del tercer trimestre. Nuestro centro suprimió dicha visita obstétrica y obtuvo datos propios para comparar los resultados perinatales logrados con dicho manejo. Método: Se realizó un estudio de cohortes retrospectivo, en marzo de 2020, con una cohorte con visita presencial única en la semana 40 de gestación (122 gestantes) frente a una cohorte con seguimiento convencional con visita presencial en la semana 36 de gestación (162 gestantes). Se evaluaron la restricción del crecimiento fetal, la edad gestacional al nacimiento, el peso neonatal y las tasas de inducciones, partos eutócicos y cesáreas urgentes en trabajo de parto. Resultados: Se encontraron diferencias leves en la tasa de nuliparidad (p < 0,04), sin hallarlas en el resto de las variables maternas. No hubo diferencias entre las dos cohortes en los resultados neonatales. Conclusiones: No hay diferencias entre los resultados materno-fetales obtenidos en gestantes con seguimiento gestacional con restricción de la visita del tercer trimestre respecto del seguimiento tradicional, excepto en el diagnóstico de las alteraciones de la estática fetal al término de la gestación.


Abstract Objective: The SARS-CoV-2 pandemic has forced a reorganization of face-to-face visits, for this reason they have been minimized to the point of reconsidering the completion of the third trimester visit. Our center eliminated the performance of this obstetric visit and obtained its own data to compare the perinatal results obtained with such management. Method: A retrospective cohort study was carried out in March 2020, with a cohort with a single face-to-face visit at 40th week of gestation (122 pregnant women), versus a cohort with conventional follow-up with face-to-face visit at 36th week of gestation (162 pregnant women). The following were evaluated fetal growth restriction, gestational age at birth, neonatal weight, rate of inductions, of eutocic deliveries, and of urgent cesarean sections in labor. Results: Slight differences were found in the nulliparity rate (p < 0.04), without finding them in the rest of the maternal variables. There were no differences between the two cohorts in neonatal outcomes. Conclusions: There were no differences between the maternal-fetal results obtained in pregnant women with gestational follow-up with restriction of the third trimester visit compared to traditional follow-up, except in the diagnosis of alterations in fetal statics at the end of pregnancy.

ABSTRACT

Objetivo: analisar quatro casos de emergências obstétricas assistidas em uma unidade de terapia intensiva adulto de um hospital do interior do Brasil. Método: estudo qualitativo, do tipo estudo de casos múltiplos, envolvendo quatro mulheres com emergência obstétrica de um hospital da Fronteira Oeste do Rio Grande do Sul, no período entre 2016 e 2018. O instrumento de coleta resgatou as características sociais e clínicas, segundo seus prontuários. A análise temática de dados foi baseada nas perguntas e confronto com a literatura. Resultados: as mulheres apresentaram os diagnósticos de acidente vascular cerebral hemorrágico, eclâmpsia, tromboembolia pulmonar e sepse. Faixa etária entre 28 e 44 anos, sendo três gestantes e uma puérpera. Tempo de espera para internação de um dia para todas, e tempo médio de permanência foi de seis dias. Conclusões: o estudo permite auxílio na construção de protocolos e na articulação das redes de atenção à gestação de alto risco.(AU)


Objective: the objective was to analyze four cases of obstetric emergencies assisted in an adult intensive care unit of a hospital in the interior of Brazil. Method: qualitative study, of the multiple case study type, involving four women with obstetric emergency of a hospital in Rio Grande do Sul, in the period between 2016 and 2018. The collection instrument rescued the social and clinical characteristics, according to their medical records. Thematic analysis of data was based on questions and comparison with the literature. Results: the women presented the diagnoses of hemorrhagic stroke, eclampsia, pulmonary thromboembolism and sepsis. Age group between 28 and 44 years old, being three pregnant women and one puerperal woman. Waiting time for admission of one day for all, and average length of stay of six days. Conclusions: the study allows assistance in the construction of protocols and in the articulation of care networks for high-risk pregnancy.(AU)

El instrumento de recolección rescatado las características sociales y clínicas, según sus historias clínicas. El análisis temático de los datos se basó en preguntas y comparación con la literatura. Resultados: las mujeres presentaron los diagnósticos de ictus hemorrágico, eclampsia, tromboembolismo pulmonar y sepsis. Grupo de edad entre 28 y 44 años, siendo tres gestantes y una puérpera. Tiempo de espera para ingreso de un día para todos, y estancia media de seis días. Conclusiones: el estudio permite auxiliar en la construcción de protocolos y en la articulación de redes de atención al embarazo de alto riesgo.(AU)

ntroduction: The acceptability and willingness to undergo Transvaginal Sonography by the patients havegeneratedmixedreactionsindifferenthealthcare settings. There is little that is known aboutZimbabweanwomen'sperceptionsandwillingness to undergo Transvaginal Sonography and there are no specific guidelines to guide its use. Aim:To evaluate the perception and willingness to undergo Transvaginal Sonography among women attending Obstetrics and Gynaecology clinic at SallyMugabeCentralHospitalinHarare, Zimbabwe.Methods:Across-sectionalsurveyusingastructured questionnaire was conducted between 1 and 30 June 2022. Atotal of 170 women attending the obstetrics and gynaecology clinic at Sally MugabeCentralHospitalwereselectedbyconsecutive sampling to participate in the study.Results:The majority of women (81.76%) had no previous TVS experience, with an equally large proportion (60.84%) not having seen a TVS probe before. Most of the women would prefer female sonographers (85.37%) to conduct the examination. In addition, about 58% of the participants concurred that a chaperone should be present. Less than half of the females (47.93%) were eager to have a TVS done on them. Equally, just 45% of the females said they would encourage others to have a TVS scan. The difference in willingness to undergo a TVS study stratified by employment status was statistically significant (chi-square 7.26, p = 0.03). Conclusion: Our study findings revealed that a large proportion of females had no previous TVS experience, with an equally large proportion not having seen a TVS probe before. Only a sizeable proportion of women were willing to accept TVS provided it is conducted by female sonographers and or in the presence of a chaperone. The findings underscoretheimportanceofeducationandawareness of the benefits of TVS in terms of diagnosis on maternal health outcomes, in the Zimbabwean population.

INTRODUCCIÓN: La pandemia de COVID-19 ha comportado una disminución de la consulta ginecológica al servicio de urgencia. OBJETIVO: Caracterizar las consultas en el servicio de urgencia ginecológica, evaluando la cantidad de hospitalizaciones, la necesidad de intervención quirúrgica y los factores de morbilidad, entre otros, comparado a la situación con la previa a la pandemia. MÉTODO: Se realizó un estudio de cohorte no concurrente de pacientes hospitalizadas tras una consulta espontánea al servicio de urgencia ginecológica entre las semanas 12 y 28 de 2019 y 2020 en el Hospital Clínico de la Pontificia Universidad Católica, en Santiago (Chile). Se determinaron distintos factores de morbilidad: diagnóstico de ingreso, intervención quirúrgica, complicación operatoria, días de hospitalización, hemoglobina/hematocrito de ingreso y necesidad de transfusión de unidades sanguíneas. RESULTADOS: Hubo 511 consultas al servicio de urgencia entre las semanas 12 y 28 del año 2019, en comparación con 196 el año 2020. En 2019 fueron hospitalizadas 103 mujeres, y en 2020 ingresaron 72 (odds ratio [OR]: 2.3). Disminuyó el ingreso por aborto retenido (24 vs. 12; p = 0.01), mientras que aumentó el ingreso por metrorragia posmenopáusica (9 vs 22; p = 0.01). No hubo cambio en las intervenciones quirúrgicas realizadas (86.4% vs. 84.7%; p = 0.7). Aumentaron los días de hospitalización (2.3 vs. 3.1; p < 0.0001) y la necesidad de transfusión sanguínea (2 vs. 7; p = 0.02; OR: 5.4; intervalo de confianza: 1.09-26). CONCLUSIONES: La pandemia de COVID-19 provocó una disminución en la consulta espontánea por patología ginecológica al servicio de urgencia, provocando un aumento en la relación consulta/ingreso y una mayor morbilidad en las pacientes hospitalizadas, caracterizada por una mayor necesidad de transfusión sanguínea y un aumento de los días de hospitalización.


INTRODUCTION: The COVID-19 pandemic has meant a decrease in gynecological visits to the emergency department. OBJECTIVE: To characterize the attending in the gynecological emergency service, evaluating the amount of admissions, hospitalizations, need for surgical intervention, morbidity factors, among others, compared to the pre-pandemic situation. METHOD: A non-concurrent cohort study of hospitalized patients was carried out through a spontaneous attending to the gynecological emergency service between weeks 12 and 28 of 2019 and 2020 at the Hospital Clínico of the Pontificia Universidad Católica, in Santiago (Chile). Different morbidity factors were measured: admission diagnosis, surgical intervention, surgical complication, hospital stay, admission hemoglobin, and need for transfusion of blood units. RESULTS: A total of 511 visitors to the emergency service were registered between weeks 12 and 28 of 2019, compared to 196 in 2020. In 2019 and 2020, 103 and 72 women were hospitalized respectively (odds ratio [OR]: 2.3). Admission for pregnancy loss decreased (24 vs. 12; p = 0.01), while admission for postmenopausal bleeding increased (9 vs. 22; p = 0.01). There was no change in the surgical interventions performed (86.4% vs. 84.7%; p = 0.7). The hospital stays increased (2.3 vs. 3.1; p < 0.0001) and the need for blood transfusion (2 vs. 7; p = 0.02; OR: 5.4; confidence interval: 1.09-26). CONCLUSIONS: The COVID-19 pandemic caused a decrease in spontaneous attending for gynecological pathology at the emergency service, causing an increase in the attend/admission ratio and greater morbidity in hospitalized patients characterized by a greater need for blood transfusion and an increase on the hospital stay.

RESUMEN Introducción: En el servicio de Obstetricia del Hospital General Docente "Dr. Agostinho Neto" de Guantánamo no se ha precisado cómo ha sido la morbilidad materna en el periodo 2015 - 2019. Objetivo: Caracterizar la morbilidad materna en el servicio de Obstetricia y la Unidad de Cuidados Intensivos del Hospital General Docente "Dr. Agostinho Neto" de Guantánamo durante el periodo 2015 - 2019. Método: Se realizó un estudio descriptivo, transversal, y retrospectivo en todas las pacientes obstétricas ingresadas en el periodo antes mencionado (N=733). Se identificaron en estas las variables sociodemográficas, clínicas y las relacionadas con los criterios de morbilidad materna extrema. La información para el desarrollo del estudio se obtuvo de la base de datos de egresos del servicio de Obstetricia, los registros estadísticos del hospital y las historias clínicas de cada gestante. Resultados: Ingresaron 733 pacientes, el 100 % por la necesidad de manejo en la unidad. La preeclampsia-eclampsia (33,6 %) fue la enfermedad más frecuente. El 71,7 % requirió tratamiento quirúrgico y, de estas, se reintervino el 29,8 %. La histerectomía obstétrica fue la técnica quirúrgica más usual (62,9 %). Conclusiones: Se caracterizó la morbilidad materna en este servicio de salud, lo que revela que la letalidad es baja pero la frecuencia es notable.


ABSTRACT Introduction: In the obstetric care service, at the Hospital General Docente "Dr. Agostinho Neto", it has not been specified how maternal morbidity index has behaved during the period 2015-2019. Objective: To characterize the maternal morbidity index in the obstetric care service and the intensive care unit (ICU) at the Hospital General Docente "Dr. Agostinho Neto", in Guantanamo, from 2015 through 2019. Method: A descriptive, cross-sectional, and retrospective study was conducted to all admitted obstetrics patients (N=733). The sociodemographic, clinical and those related to the criteria for extreme maternal morbidity were the baseline variables identified. The information used in the study was acquired from the updated obstetric care service database (discharged patients database), the hospital's statistical records and the history of each pregnant woman. Results: The total of obstetrics patients (N=733) were admitted, it was a priority to be manage for the ICU. Preeclampsia-eclampsia was the most frequent disease (33.6%). Surgical treatment was required in the 71.7% and 29.8% of them needed to re-intervene. The hysterectomy was the most common surgical technique used (62.9%). Conclusions: The maternal morbidity in the obstetric care service was successfully characterized, which reveals that the lethality is low but the frequency is remarkable.


RESUMO Introdução: No serviço de Obstetrícia do Hospital General Docente "Dr. Agostinho Neto" de Guantánamo não foi especificado como está a morbidade materna no período 2015-2019. Objetivo: Caracterizar a morbidade materna no serviço de Obstetrícia e na Unidade de Terapia Intensiva do Hospital General Docente "Dr. Agostinho Neto" de Guantánamo durante o período 2015-2019. Método: Foi realizado um estudo descritivo, transversal e retrospectivo em todas as pacientes obstétricas admitidas no período mencionado (N=733). Neles foram identificadas variáveis sociodemográficas, clínicas e aquelas relacionadas a critérios de morbidade materna extrema. As informações para o desenvolvimento do estudo foram obtidas no banco de dados de altas do serviço de Obstetrícia, nos prontuários estatísticos do hospital e nos prontuários de cada gestante. Resultados: Foram internadas 733 pacientes, 100% devido à necessidade de gerenciamento na unidade. A pré-eclâmpsia-eclâmpsia (33,6%) foi a doença mais comum. 71,7% necessitaram de tratamento cirúrgico e, destes, 29,8% foram reoperadas. A histerectomia obstétrica foi a técnica cirúrgica mais comum (62,9%). Conclusões: A morbidade materna foi caracterizada neste serviço de saúde, o que revela que a mortalidade é baixa mas a frequência é notável.

ABSTRACT

Prólogo. La obra recopiló los hitos históricos, las acciones en gestión y la visión futura de las Unidades Médicas, Administrativas y de Enfermería, en homenaje a los 50 años de función del Hospital de Especialidades Carlos Andrade Marín como ícono de la Seguridad Social del Ecuador, entre el periodo 1970 a diciembre 2020. Cada autor registró el título entrelazado un pensamiento, previo a la cronología de los hechos suscitados en los servicios brindados en beneficio de los pacientes que se atendieron en esta casa de salud. La obra se clasificó en capítulos por Unidades Médicas: Críticas, Clínicas, Quirúrgicas, Administrativas y de Enfermería, se incluyeron tablas, figuras, producción médica, patologías, cambios de técnicas o procedimientos y sustentó su accionar con citas textuales y referen-cias bibliográficas. Además, se narraron mejoras continuas transmitidas de los ilustres y legados maestros, especialistas médicos por varias generaciones, en conjunto con el personal de salud con vocación de servicio, en una reflexión profunda con mensajes vivos de su accionar. En las modalidades de publicación se realizaron aportes de visión nacional e internacional en los ámbitos asistenciales, administrativos, docente, de investigación y producción científica de alto nivel del contexto y naturaleza del emblemático hospital. Finalmente, se agradece a los funcionarios y ex trabajadores que colaboraron durante el proceso de construcción de la obra, así como a ex colaboradores que motivaron aportes de experiencias y la apreciación personal.


Prologue. The work compiled the historical milestones, management actions and future vision of the Medical, Administrative and Nursing Units, in tribute to the 50 years of function of the Carlos Andrade Marin Specialty Hospital as an icon of the Social Security of Ecuador, between 1970 and December 2020. Each author recorded the title intertwined with a thought, prior to the chronology of the events that took place in the services provided for the benefit of patients who were treated in this health center. The work was classified in chapters by Medical Units: Critical, Clinical, Surgical, Administrative and Nursing, and included tables, figures, medical production, pathologies, changes in techniques or procedures, and supported his actions with textual quotations and bibliographical references. In addition, continuous improvements were narrated and transmitted from the illustrious and legacy masters, medical specialists for several generations, together with the health personnel with vocation of service, in a deep reflection with living messages of their actions. In the publication modalities, contributions of national and international vision were made in the care, administrative, teaching, research and scientific production of high level of the context and nature of the emblematic hospital. Finally, we would like to thank the employees and former workers who collaborated during the construction process, as well as former collaborators who contributed with their experiences and personal appreciation.

INTRODUCCIÓN: La red de atención de urgencia es fundamental en la salud de cualquier país. En Chile, los servicios de urgencia periódicamente están saturados por usuarios con patologías de carácter leve; lo mismo ocurre en las atenciones de urgencias gineco-obstétricas (UGO) en los centros de mediana y alta complejidad. OBJETIVO: Describir la categorización de las consultas de urgencias gineco-obstétricas en un hospital público de Chile, atendidas durante el año 2018. METODOLOGÍA: Estudio descriptivo y transversal que abarcó 3.077 consultas obtenidas de datos estadísticos anonimizados. RESULTADOS: De las UGO, el 58,3% fueron consultas obstétricas, el 36,9% ginecológicas y 4.8% neonatológicas; el 96% fueron consultantes mujeres; el 80,3% eran adultos y el 11,6% adolescentes; el 92,5% eran usuarios pertenecientes a la comuna de Penco; el mes más consultado fue mayo con 292 atenciones y el 71,5% de las consultas fueron atendidas en horario diurno. Los principales motivos de consulta obstétrica y ginecológica fueron el dolor y el sangramiento genital; en las urgencias neonatológicas, la ictericia fue el principal motivo de consulta. El diagnóstico más frecuente en las consultas obstétricas según clasificación CIE _10 fue la supervisión de embarazo normal, en las ginecológicas fue el dolor abdomino-pélvico y en las consultas neonatológicas, la ictericia; del total de consultas UGO un 91,1% fueron resueltas a nivel local. El 0,1% de las UGO fueron categorizadas como c1 y el 0,3% como c2, siendo la gran mayoría, consultas de baja complejidad. CONCLUSIONES: Existe un gran porcentaje de consultas de baja complejidad que sobrecargan el servicio de urgencia, afectando la calidad de la atención del hospital.


INTRODUCTION: the urgent care network is fundamental in the health of any country. In Chile, the emergency services are periodically saturated with users with mild pathologies; the same occurs in gynecological-obstetric emergency care (UGO) in more complex centers. OBJECTIVE: to describe the categorization of gynecological-obstetric emergency consultations in a public hospital in Chile, attended in 2018. METHODOLOGY: descriptive and cross-sectional study; covered 3,077 queries of anonymized statistical data. RESULTS: of the UGO, 58.3% were obstetric consultations, 36.9% gynecological and 4.8% neonatological; 96% were women; 80.3% were adults and 11.6% adolescents; 92.5% were from Penco; the month most consulted was may with 292 visits and 71.5% of the consultations were during daytime hours. The main reasons for obstetric and gynecological consultation were pain and genital bleeding; in neonatal emergencies, jaundice was the main reason for consultation. The most repeated diagnosis in obstetric consultations was the supervision of normal pregnancy; in gynecological cases, it was abdominal-pelvic pain and in neonatological consultations, jaundice; there was 91.1% local resolution. 0.1% of the UGO were categorized as c1 and 0.3% as c2, the vast majority being low complexity queries. CONCLUSIONS: there is a large percentage of low complexity consultations that overload the emergency service, affecting the quality of hospital care.

ABSTRACT

Introdução: Potenciais riscos aos pacientes são intrínsecos à assistência à saúde, sendo que na maternidade, a ocorrência de eventos adversos pode repercutir em danos a mulher e ao recémnascido (RN). Diante disso, políticas públicas estabelecem ações de melhoria da assistência por meio da corresponsabilização e participação ativa dos pacientes e acompanhantes no cuidado e na prevenção de incidentes. Nesse contexto, a literatura sugere o desenvolvimento de tecnologias educativas (TE) para aperfeiçoar a comunicação, aumentar a capacidade de escuta e engajar o paciente e família na segurança do paciente (SP). Entretanto, as evidências sobre o desenvolvimento e utilização de TE em busca do maior envolvimento do acompanhante e do paciente na SP na maternidade, especialmente com a participação desses atores são insuficientes. Objetivos de pesquisa: Conhecer as percepções e as experiências de pacientes, acompanhantes e profissionais de saúde da maternidade sobre a participação da paciente e do acompanhante na segurança da paciente e do RN. Objetivo de prática: Elaborar e avaliar uma tecnologia educativa para promoção do envolvimento das pacientes internadas na maternidade e seus acompanhantes nas ações do cuidado seguro. Método: O estudo seguiu os preceitos metodológicos da Pesquisa Convergente-Assistencial e sustentou-se nos fundamentos teóricos de Paulo Freire. Foi realizado na maternidade de um hospital público de Belo Horizonte e a produção de dados foi dividida em três etapas. A primeira etapa subsidiou a construção da cartilha através de entrevistas para conhecer as percepções e o conhecimento dos participantes sobre a SP e a participação da paciente e do acompanhante na segurança; e identificar as barreiras que dificultam colocar em prática as ações de SP. Participaram dessa etapa 13 profissionais de saúde, 11 pacientes e 06 acompanhantes. Na segunda foi desenvolvida a TE em forma de cartilha e na terceira, a mesma foi avaliada pelos participantes. Na última etapa aceitaram continuar participando da pesquisa os 13 profissionais da saúde, 08 pacientes e 05 acompanhantes. A coleta de dados ocorreu por meio de entrevista semiestruturada e registro de diário de campo. A análise dos dados foi realizada pela análise de conteúdo e foram criadas duas categorias provenientes da primeira etapa: Saberes e experiências sobre a segurança da paciente e do RN na maternidade e Desafios para o envolvimento da paciente e acompanhante nas ações de segurança. Resultados: Na primeira categoria os achados evidenciaram que as pacientes e os acompanhantes esboçaram diferentes entendimentos sobre a SP, sendo que a maioria apresentava compreensão limitada sobre o tema e desconhecia como poderiam contribuir para SP. Os profissionais enfatizaram que as pacientes e os acompanhantes se envolvem pouco na SP e poderiam ser mais informados e estimulados pela própria equipe. A segunda categoria apontou as fragilidades para a participação da paciente e acompanhante nas ações de segurança, como a falta de conhecimento, diálogo e escuta, e a posição autoritária de alguns profissionais. Os achados da primeira etapa contribuíram para a construção da TE, no formato de cartilha. Na terceira etapa, os participantes avaliaram a TE como sendo um material importante para impulsionar a participação das pacientes e acompanhantes na SP. Conclusão: O processo interativo e dialogado com os participantes possibilitou a criação da TE como ferramenta importante no envolvimento das pacientes e acompanhantes nas ações de SP. Essa experiência vai ao encontro do pensamento de Paulo Freire que afirma que, o educador deve reconhecer o educando têm saberes, experiências e leituras de mundo próprias, sendo o respeito essencial para influenciar no desenvolvimento de novos saberes.


Potential risks to patients are intrinsic to health care, and in maternity hospitals, the occurrence of adverse events can affect women and newborns (NB). Therefore, public policies establish actions to improve care through co-responsibility and active participation of patients and caregivers in the care and prevention of incidents. In this context, the literature suggests the development of educational technologies (ET) to improve communication, increase listening skills and engage the patient and family in patient safety (PS). However, the evidence on the development and use of ET in search of greater involvement of the companion and the patient in PS in the maternity hospital, especially with the participation of these actors, is insufficient. Research objectives: Know the perceptions and experiences of patients, companions and health professionals in the maternity ward about the participation of the patient and companion in the safety of the patient and the NB. Practice objective Develop and evaluate an educational technology to promote the involvement of patients admitted to the maternity ward and their companions in safe care actions. Method: The study followed the methodological precepts of the Convergent Care Research and was supported by Paulo Freire's theoretical foundations. It was carried out in the maternity of a public hospital in Belo Horizonte and data production was divided into three stages. The first stage supported the construction of the booklet through interviews to learn about the participants' perceptions and knowledge about PS and the patient's and companion's participation in safety; and identify the barriers that make it difficult to put PS actions into practice. Thirteen health professionals, 11 patients and 06 caregivers participated in this stage. In the second, the ET was developed in the form of a booklet and in the third, it was evaluated by the participants. In the last stage, the 13 health professionals, 08 patients and 05 companions agreed to continue participating in the research. Data collection took place through semi-structured interviews and field diary records. Data analysis was performed using content analysis and two categories were created from the first stage: Knowledge and experiences about patient and newborn safety in the maternity ward and Challenges for the involvement of the patient and companion in safety actions. Results: In the first category, the findings showed that patients and caregivers outlined different understandings about PS, and most of them had limited understanding of the topic and did not know how they could contribute to PS. Professionals emphasized that patients and companions are little involved in PS and could be more informed and encouraged by the team itself. The second category pointed out the weaknesses for the participation of the patient and companion in safety actions, such as the lack of knowledge, dialogue and listening, and the authoritarian position of some professionals. The findings of the first stage contributed to the construction of the ET, in the form of a booklet. In the third stage, participants assessed ET as being an important material to boost the participation of patients and caregivers in the PS. Conclusion: The interactive process and dialogue with the participants enabled the creation of ET as an important tool in the involvement of patients and caregivers in PS actions. This experience is in line with the thought of Paulo Freire who states that the educator must recognize that the student has their own knowledge, experiences and readings of the world, with respect being essential to influence the development of new knowledge.

RESUMEN El gran desarrollo del programa materno infantil en Cuba y la gran atención que a este se brinda hace que muchos médicos limitan a la embarazada al reposo y todavía hay muchas mujeres que se enfrentan al parto sin un entrenamiento previo. Por la importancia de este tema en materia de salud materno infantil se realizó esta revisión bibliográfica con el propósito de aumentar el nivel de conocimientos de los profesionales sobre los beneficios de la misma y lograr un trabajo en equipo del personal que integre el programa, para de esta manera, conseguir el máximo beneficio para las madres, bebés y sus familias. Está demostrado que le hace bien a la mujer embarazada y al bebé. Una gestante preparada es una gestante colaboradora. Por tanto, el entrenamiento junto a hábitos sanos de vida conllevara a que la embarazada tenga un parto en las mejores condiciones físicas y psíquicas, para de esta forma lograr un hijo sano, fuerte y una gran gratificación para toda la familia. La participación activa del padre conseguirá mayor armonía familiar (AU).


SUMMARY The great development of the maternal-infantile program in Cuba and the great attention provided to this, leads to the limitation of pregnant women to rest by doctors, and there are still many women facing the childbirth without a previous training. Due to the importance of this topic in the matter of maternal-infantile health, the authors carried out the current bibliographical review with the purpose of increasing the level of the professionals' knowledge about its benefits and of achieving a team work of the staff integrating the program, and that way, to get the maximum benefit for mothers, babies and their families. It is proved that it makes well to pregnant woman and to the baby. A trained pregnant woman is a collaborative pregnant woman. Therefore, training together with healthy life habits leads pregnant women to childbirth in the better physical and health conditions, delivering this way a healthy, strong child and achieving a great gratification for the whole family. The father's active participation will get bigger family harmony (AU).

RESUMEN Patriota, médico y científico cubano, especialista en Obstetricia y Ginecología, Eusebio Hernández Pérez fue combatiente de las tres guerras del siglo xix por la independencia de Cuba. Como hombre de ciencias obtuvo notables reconocimientos y dejó una apreciable obra escrita en el terreno de la Obstetricia y la Ginecología. Fue un destacado revolucionario cubano, historiador y partícipe de los hechos narrados, y un excepcional profesor de Obstetricia y Ginecología de la Universidad de la Habana durante 30 años (AU).


SUMMARY Cuban patriot, physician and scientist, specialist in Obstetrics and Gynecology, Eusebio Hernandez Perez was fighter of the three wars for Cuban independence in the XIX century. As a man of sciences he got remarkable recognitions and led a substantial written work in the field of Obstetrics and Gynecology. He was an outstanding Cuban revolutionary, historian and participant of the events recounted and an exceptional professor of Obstetrics and Gynecology of the University of Havana for thirty years (AU).

INTRODUCCIÓN Y OBJETIVOS: El Síndrome Respiratorio Agudo Grave Coronavirus 2 (SARSCoV-2) es una enfermedad altamente contagiosa y que puede ser transmitida por pacientes asintomáticos. Por esto surge el interés de poder determinar la prevalencia de la infección por SARS-Cov-2 en pacientes embarazadas que ingresan para interrupción de la gestación. MÉTODOS: Se realizó un análisis descriptivo, retrospectivo en el Servicio de Obstetricia y Ginecología de Hospital de Carabineros de Chile entre el 15 de mayo y el 30 junio del 2020. Se incluyeron todas las mujeres embarazadas que ingresaron para interrupción de la gestación, a las que se les realizó el examen PCR SARS-CoV-2; y una encuesta de signos y síntomas sugerentes de la enfermedad. RESULTADOS: Se realizaron 73 interrupciones de la gestación, con toma de PCR a 72 mujeres; de estas pacientes 65 (90.3%) fueron negativas, 5 (6.9%) positivas y 2 (2.8%) indeterminadas; los resultados indeterminados fueron considerados como positivos, por lo que la prevalencia de positividad fue de 9,5%. De estas pacientes, sólo 1 de ellas tenía síntomas sugerentes de la enfermedad, todas las demás (6) eran pacientes asintomáticas, y se mantuvieron así durante toda la hospitalización. CONCLUSIÓN: La realización del examen PCR para SARS-CoV-2 a todas las embarazadas que ingresan a un servicio de Ginecología y Obstetricia ayuda a identificar a las pacientes asintomáticas contagiadas con el virus. Ya que la consulta por presencia de signos y síntomas no permite identificar los casos positivos, es necesario considerar la realización de este examen en los protocolos de ingreso hospitalario a lo largo de nuestro país.


INTRODUCTION AND OBJECTIVES: Severe Acute Respiratory Syndrome Coronavirus (SARSCoV-2) is a highly contagious disease that can be transmitted by asymptomatic patients. Therefore, is of interest to determine the prevalence of SARS-Cov-2 infection in pregnant patients entering for interruption. METHODS: A descriptive, retrospective analysis was performed in the Obstetrics and Gynecology Service of the Hospital de Carabineros de Chile between May 15 and June 30, 2020. Pregnant women who entered for interruption of their pregnancy and who were given the SARS-CoV-2 PCR exam were included. A survey of signs and symptoms suggestive of the disease was applied. RESULTS: There were 73 pregnancy interruptions, 72 of them were tested by SARS-CoV-2 PCR exam. Among these patients, 65 (90.3%) resulted negative, 5 (6.9%), were positive and 2 (2.8%) were indeterminate; indeterminate results were considered positive, so the prevalence of positivity was 9.5%. Of these patients only 1 had symptoms suggestive of the disease, all the others (6) were asymptomatic, and remained so throughout the hospitalization. CONCLUSION: Conducting the PCR test for SARS-CoV-2 for all pregnant women entering a Gynecology and Obstetrics service helps to identify asymptomatic patients infected with the virus. As a survey of signs and symptoms cannot identify positive patiens, it is necessary to consider conducting universal screeing in hospital admission protocols throughout our country.

INTRODUCCIÓN: La infección por el coronavirus SARS-CoV2 (COVID 19), causal de la pandemia actual, ha significado a nivel mundial la hospitalización simultánea de múltiples pacientes poniendo a prueba la infraestructura hospitalaria y la capacidad de reacción del personal de salud. Una de las estrategias para el manejo es la reconversión de camas y servicios clínicos. OBJETIVOS: presentar experiencia de un equipo ginecológico en el manejo integral de pacientes no gineco-obstétricas con COVID 19, durante el mes de junio de 2020 en un hospital público de la Región Metropolitana. MÉTODOS: Estudio de corte transversal observacional, descriptivo. Se consideró el total de pacientes adultos hombres y mujeres sin patología gineco-obstétrica con COVID 19 ingresados al puerperio del Hospital Santiago Oriente, obteniéndose datos clínicos y demográficos a través del registro interno de la unidad y del sistema de información de red asistencial. RESULTADOS: Ingresaron 82 pacientes, 32 mujeres y 50 hombres, promedio de edad 64. El promedio de días de hospitalización fue 5, con diagnóstico de ingreso principal neumonía viral por COVID-19. Las comorbilidades frecuentes fueron hipertensión arterial sistémica y diabetes mellitus. La complicación más frecuente fue el tromboembolismo pulmonar agudo. Hubo una alta cobertura de entrega de información vía telefónica a familiares. De los 82 ingresos, 54 pacientes egresaron a su domicilio. El resto a otras unidades dentro de la institución, centros de menor complejidad o residencias sanitarias. Una paciente sexo femenino de 75 años fallece a causa de descompensación de patologias de base secundario a neumonia por Staphylococus aereus. En ella, se descarta la infección por COVID 19 dado tres exámenes por reacción de polimerasa en cadena negativos realizado antes y durante su hospitalización. CONCLUSIONES: Esta experiencia constituyó un desafío para todo el equipo de salud gineco-obstétrico, considerando que nos enfrentamos a otro tipo de pacientes y a una patología nueva. Los resultados médicos son promisorios, la experiencia humana y sentido de trabajo en equipo fue extraordinario.


INTRODUCTION: The infection by the SARS-CoV2 coronavirus (COVID 19), the cause of the current pandemic we are experiencing, has meant the simultaneous hospitalization of many patients worldwide, putting the hospital infrastructure and the reaction capacity of health personnel to the test. One of the management strategies is the reconversion of clinical services. OBJECTIVES: present the experience of a gynecological team in the comprehensive management of non-gyneco-obstetric patients with COVID 19, during the month of June 2020 in a public hospital in the Metropolitan Region. METHODS: descriptive, observational cross-sectional study. The total number of patients admitted to the ex-puerperium of the Santiago Oriente Hospital was considered, obtaining clinical and demographic data through the unit's internal registry and the healthcare network information system. RESULTS: 82 patients were admitted, 32 women and 50 men, average age 64. The average number of days of hospitalization was 5, with the main admission diagnosis being viral pneumonia due to COVID-19. Frequent comorbidities were systemic arterial hypertension and diabetes mellitus. The most frequent complication was acute pulmonary thromboembolism. There was a high coverage of the delivery of information via telephone to relatives. Of the 82 admissions, 54 patients were discharged home and the rest to other units within the institution, less complex centers or health residences. One 75 years old female patient dies from concomitant pathologies, and she wasn't positive for COVID-19. CONCLUSIONS: This experience was a challenge for the entire gynecological-obstetric health team, considering that we are facing other types of patients and a new pathology. The medical results are promising, the human experience and sense of teamwork was extraordinary.

ABSTRACT

La pandemia por COVID-19 ha generado un estancamiento mundial en la atención y resolución de todas las patologías graves y crónicas, debido al colapso de los sistemas de salud, a la dificultad de consulta, dada por la disminución de movilidad de las personas, por las cuarentenas establecidas y también por el temor de los pacientes al contagio en los centros de salud. Los enfermos oncológicos han visto canceladas sus atenciones, suspendidos o postergado sus tratamientos y diferidas sus cirugías. Esto no ha sido ajeno a las pacientes con cáncer de mama. En el presente trabajo, se reporta la experiencia de una Unidad de Patología Mamaria de un hospital público de Santiago de Chile y de las acciones realizadas para mantener la continuidad de atención en una comuna con una alta tasa de infección por SARS-CoV-2.


The COVID-19 pandemic has generated a global stagnation in the care and medical treatment of serious and chronic illnesses due to the collapse of the health systems, the difficulty of consulting due to decreased mobility caused by forced quarantines and also because of the fear of infection when attending a health center. Cancer patients have had their medical appointments canceled, their treatments suspended or postponed, and their surgeries delayed. Breast cancer patients have not been the exception. We report the experience of a Breast Pathology Unit of a public hospital in Santiago of Chile, and the actions carried out to maintain continuity of care in a community with a high infection rate of SARS-CoV-2.

ABSTRACT

Objetivo: Determinar la calidad de atención del parto según puérperas del Servicio de obstetricia del Hospital Regional de Ica, Mayo -Setiembre 2019. Materiales y métodos: Estudio descriptivo, observacional, prospectivo y transversal, muestra de estudio conformada por 211 puérperas, aplicando como técnicas la entrevista y encuesta, como instrumento de recolección de datos el cuestionario SERVPERF de calidad de atención, medido en una escala de 1 a 6 según satisfacción percibida, validado internacionalmente y ampliamente utilizado en la atención del parto. Resultados: La calidad de atención del parto, según satisfacción de las puérperas es en promedio de nivel muy bueno X =5.16, evidenciando que 70.1% percibe la calidad de atención de nivel muy bueno (70.1%), nivel bueno 26.1%, nivel malo 3.3% y nivel extremadamente bueno 0.5%. Por dimensiones la calidad de atención del parto es de nivel muy bueno para cada una de ellas, siendo las dimensiones mejores valoradas los elementos tangibles X = 5.43, la empatía X = 5.32 y la confianza X = 5.18; y menos valoradas la dimensión respuesta rápida X = 5.05 y seguridad X = 5.04. Se encontró asociación estadística significativa entre las variables sociodemográficas: edad (p 0.013), grado de instrucción (p 0.000), y estado civil (p 0.024). Las variables no asociadas fueron la ocupación (p 0.197), procedencia (p 0.475), y personal que atendió el parto (p 0.836). Conclusiones: La calidad de atención del parto, según satisfacción de las puérperas atendidas es de nivel muy bueno, existiendo un 3.3% de insatisfacción o nivel malo de calidad. (AU)


Objective: Determine the quality of childbirth care according to postpartum women of the Obstetrics Service of the Regional Hospital of Ica, May-September 2019. Materials and methods: Descriptive, observational, prospective and cross-sectional study, study sample formed by 211 postpartum, using the interview and survey techniques as techniques, as a tool for data collection, the SERVPERF quality of care questionnaire, measured on a scale of 1 to 6 according to perceived satisfaction, internationally validated and widely used in childbirth care. Results: The quality of delivery care, according to the satisfaction of the postpartum women, is on average very good level X = 5.16, showing that 70.1% perceive the quality of care at a very good level (70.1%), good level 26.1%, level Bad 3.3% and extremely good level 0.5%. By dimensions, the quality of childbirth care is of a very good level for each of them, the tangible elements being tangibles = X = 5.43, the empathy X = 5.32 and the confidence X = 5.18; and the value of the rapid response dimension X = 5.05 and safety X = 5.04 are less valued. A significant statistical association was found between the sociodemographic variables: age (p 0.013), degree of instruction (p 0.000), and marital status (p 0.024). Non-associated variables were occupation (p 0.197), origin (p 0.475), and personnel who attended the delivery (p 0.836). Conclusions: The quality of childbirth care, according to satisfaction of the treated women is of a very good level, with 3.3% of dissatisfaction or poor level of quality. (AU)
